apology for the delayed response.. “Quick forms option” is available with “advance custom field” add-on.
To add extra input fields you have to install “advance custom field”

Please Update “Pro Membership” add-on on your site.
Go to your download manager settings page, select quick forms and create a form with form name,
then enable email lock and On “Additional Fields” select the form you want to add with email lock.

Please set the value of order_by “publish_date” or “update_date”
And in the ‘cols’ you have to use the exact same value.

i have created a page “TEst Page” on your site and [wpdm_all_packages] , [wpdm_archive_filter] both the shortcodes working properly.
Is it possible to change the site theme temporarily for testing purpose ?

You can also check in your order details page if the license key is linked to another domain. In that case, just unlock the key ( you’ll see the option in popup ) from there and save the key in current site.
You can control the view from “Category Page Template”

Download pages are opening fine now, the issue was due to mis-configuration of UI settings. I’ve simply updated WPDM UI settings and everything are fully functional now.
