Please Update “Pro Membership” add-on on your site.
Go to your download manager settings page, select quick forms and create a form with form name,
then enable email lock and On “Additional Fields” select the form you want to add with email lock.

Download pages are opening fine now, the issue was due to mis-configuration of UI settings. I’ve simply updated WPDM UI settings and everything are fully functional now.
