You were using the Download After Submit add-on for Liveform. We dropped the support for this add-on because Form Lock ( https://www.wpdownloadmanager.com/download/wpdm-form-lock/ ) is a better product for this use case. I have installed the Form Lock on your site and form locked downloads are working now.

2 ) No, if user downloads without login and the package is open ( not locked ) no cookies are set.
Do you have the latest version of WordPress? If not please update the WordPress version.

Divi theme’s credentials are not related to Download Manager. However, if you need help in update process send wp-admin login info in private reply. I will install the updated version there.
Sorry about the order issue. It was due to a 2CO IPN issue. It has been fixed now.
The login issue in updates tab maybe related to password. Have you changed your password on our site recently? If you think your WPDM credentials are okay please send me the info in private reply including wp-admin access to your site.
Download Manager sets the following cookies and these cookies are essential for the plugin to perform the basic functions. All of these are temporary means cookies are erased after closing the browser.
unlocked_{file_id} – Tracks whether or not the current visitor unlocked a package/download.
wpdmimported – Tracks the imported files of a directory when bulk importing files from a server dir.
wpdm_getlink_data_{key} – Remembers the package/download data of an unlocked package/download for current user.
global_success – Used to track user registration.
And about the IP logging, Download Manager logs the users IP address for each download. If you want you can disable it from settings ( from the next version which will be released today ) or add the info about this in your privacy policy to inform users.
The plugin doesn’t send any information about users to anyone else. I am assuring you using WPDM won’t get you in trouble with the law. The issue is sensitive when using user data for Advertising or sharing with third parties. None of which is done by WPDM.
We are releasing the Pro update today which will include the option.
Your Private Download Links Usage Limit: was set to 0 ( In Basic Setting Tab ). That is the number of times the URL can be used to download the file. I have set it to 5 and download is working like a charm.
