Yes, anyone can pay using any valid credit/debit card, paypal account is not required.

Hi, I’m having this issue: my client has a paypal account that is from outside the US, so this add-on doesn’t work because the client is not able to recieve direct payments… is there any way I can claim a refund or change it for another solution like PayU for example?

Sorry, I’m not sure I understand. I presume I need a Payment Gateway to be able to sell digital goods (eBooks) through the site from this list here https://www.wpdownloadmanager.com/download/payment-gateways/
The only gateway that mentions Paypal is Paypal Payments Pro.
I would also need a Paypal merchant account.
Is this correct?

This is not required for Premium Package. It is just an option. If you want PayPal Payments Pro features then purchase this add-on.
