you need to create a custom field group from wpdm settings custom fields

Now, when creating/editing a package you need to enable email lock and select the field group from wpdm package settings lock options tab, finally publish. Now at front-end when user will try to download, it will show the form and after user submits it, visitor will see a message. But for now, the message is not customizable.

Sorry, many files are changed in this update due to wp 4.5 compatibility reason.

BTW, I think, you can do that now too, you need to use category short-code with order by download_count

Actually , the update for Advanced Custom Field Add-on still not released.

Looks like a conflict with theme or any other active plugin, you need to change disabling other plugins one after another and changing theme temporarily. Or, please give me temporary wp-admin lognin info, I shall do that for you.

You can customize/create custom page  template from Admin Menu Downloads Templates , But text color is not coming from wpdm, it is coming from following css:
