You can use your own template for add new page.

You have to use the download-manager/tpls/wpdm-add-new-page-front.php template. Check this documentation for more info.

Remove ending / from your permalink settings. That should fix the url issue.

Did you customize anything in Download Manager? Looks like files are not being uploaded to the download-manager-files directory.

You are also using old version. Try installing latest version.

If updating plugin does not fix it, send FTP login info. I have to check some directory permissions.

Sorry, i think you did not understand what i meant in previous reply. All features are available in plugin you have.

User can purchase membership ( means membership is remium ) levels from frontend. But in your case, users are not purchasing membership plans. You are assigning membership to user after reviewing their profile. And you can do that from admin as shown in screenshot above.

WPDM captcha is not for registration form. You can apply reCAPTCHA lock in a package so that user has to verify captcha  before downloading the file.
