1 ) You can set the store logo from Frontend Uploader Edit Public Profile tab.

2 ) Set the custom message value in edit package page. Make sure you are using the template tag ( from WPDM Custom Fields add-on ) in your page template.
3 ) Do you want to add other share options? You can do that by editing the page template you are using. Edit page templates from Downloads Templates page.

Most users want the plugin as it is now. If you need a slightly different feature you can modify it as you want but you have to keep a backup of your modifications because the update will erase all changes.

Please keep your Premium Package custom template files in /active-theme/download-manager/ dir just like the Download Manager core plugin custom template files.
By the way, a new update of Premium Package coming soon. There are some changes in template files. So, it is best if you do the modification after the update is released.

Looks like WP Forms doesn’t allow form submission from the iframe. I have applied extended page template here http://www.latroservices.com/download/versalytics-data-sheet-download/ to embed the form on the page. Now submissions are listed in entries page.

2 ) Replace $log_redirect ( line 121 ) with $_SERVER['REQUEST_URI'] on this file /active-theme/download-manager/wpdm-login-form.php

Please send login info in private reply to check the version issue. Are you using a custom template to show the all package table?
We are releasing an update of Premium Package in next week. There will be lots of improvements in this new version. I couldn’t reproduce the issue on my dev server with the new version. So, I hope updating to the latest version will fix your issue too.
Currently, Single file download is possible only in a password locked package. Please create separate packages for each file if you need the lock in each file.
Maybe you package is locked. Individual buttons are not active in the locked package.
I have downloaded some packages and looks like files are okay. Please send me the package URL where you encountered the issue.
It was an issue with the add-on. Will add the fix in our next release too. So, the update won’t bring this back.
