Uploading New File Into Package doesn't show up

I have a number of existing packages complete. When I go in and try to add in a new pdf and delete out the old pdf, the new pdf upload shows up in the file area, I mark the old pdf to delete and update. But then after updating the old pdf is still visible and the new pdf is nowhere to be seen.

It shows up in the uploads area when I check with FTP but it doesnt’ attach to the package.

Running version 4.1.7 with accordian addon.

It is working fine in our side, so I need to check the issue in your installation, please give me temporary wp-amdin login info in a private reply.
