Showing description with file

I’ve created a file package and when inserting it into my page i want to show the title and description as well as the download icon.
When i use the toolbar option to add the file, the “Show Title” and “Show Description” tickboxes dont do anything. What format should the shortcode take to show the file title, description and download icon?
