I’m using the Responsive Table and was wondering how do I get the file icon to show up on the left like you have on the demo page?

On the shortcode you can remove thumb=1 or thumb=0 to show icons
You can select Icon from wp-admin “Package settings” section for each file on “Downloads” menu.
