How to bulk upload OR display categories (in free version)

Hi, this plugin seems to be what I want… but I can’t get it working properly.
I’d like 4 categories (1 of them can have subcategories)

There will be quite some pictures in those categories, so how can I bulk upload several files when I add a new item? I can only add 1. I also am unable to add a zip file.
In settings I’ve enabled bulk upload.

Please follow this and download the sample.csv file from there to get idea about the csv file to upload.

Bulk Import
