Good news, no conflicts with themes or plugins. The issue was related to a multi-site setting I was not familiar with for uploads. After matching the settings from my WPDM it worked.
For user experience, is there a way to provide user feedback if uploads are not the correct file type or to large? That would be helpful.
That is correct. The shortcode is just for uploading resources.
