So our hosting engineers took a look and found this in the error log:
[Mon Sep 28 12:15:25.226837 2020] [core:alert] [pid 1684] [client 172.30.9.254:42328]
/var/www/htdocs/wp-content/plugins/download-manager/.htaccess: Invalid command ‘IndexIgnore’,
perhaps misspelled or defined by a module not included in the server configuration, referer: http://predict.uk.w3pcloud.com/
They commented out the IndexIgnore line of code in htaccess and now everything is working correctly.

Thanks for getting back to me with a response.
I have tried both of your suggestions above, but unfortunately the plugin is still not working properly.
When I load any pages within this plugin backend, I receive a bunch of 500 Internal server errors in the inspect element console. It seems the plugin is failing to load js from its assets folder?
