Out of interest I just did a fresh install on a test site and in the settings it has Admistrator and the code in the Login Required Message as default so guess its not that. I have attached a screen shot of how it looks, there should be a blue Download PDF button under each brochure, the button shows up when logged in a Admin so not sure whats happening.

OK, I have switched to using a opt in plugin which works but the Download button is not appearing. In the basic settings under Access Settings > File Browser Access: it has Administrator. I guess this is it but if I delete it and Save settings (I get the settings save message) it remains set plus in the Login Required Message: I have some code rredirecting to another URL which had the login for. Again if I delete this and save settings it remains when I refresh the basic settings page. I thought it was down to Wordfence stopping the settings from saving but I have disabled it.

Any ideas?

Thanks, it is down to wordfence. Once I disable that plugin it saves fine.
