The initial post indicates that several forms WERE created first. That is the problem, they do not show up in the drop-down. The image attached to the initial post is AFTER forms have been created.

As a side note, I have decided not to use this plugin for the current project. In concept, as others have stated in these forums, WDM seems like a great product. There just seem to be to many loose ends, little bugs, and undocumented functionality. I would however like to get this problem resolved as I may try to use the plugin in future projects.

That said, I do appreciate your timely response to the initial question.
