Questions wrt email lock custom fields:
1.) where do I enter this code (what file)?
2.) If I create a field with the ID of “cust_company”, how do I insert this into that code in the body of the message?
3.) How do I insert custom field data into the template that is used to send the link to the customer? I see in Subscribers>email template that there is [download_url] and [site_name] is there something similar for custom fields? I tried [cust_company] but that doesn’t work.
4.) how do I make a custom field required?
There appears to be one new error wrt email lock actually.
After Update: email-locked downloads > message missing "link sent…"
I just updated and had no new errors.
worked. Also I am upgrading to 4.5.1 now to catch anything else.

I got some good news via email!!!
“Don’t worry, we are already adding option to email download link with form lock rather then download instantly, and update is coming within this week ๐ ”
And
“now the download link has a usage limit 3.”
These two things, form with email link, and limited use link in email solves every problem I have….

As an update to any watching, I attempted to do as Substance151 suggested and installed the advanced custom fields add on that came with the special pack I purchased. This looks promising for creating the fields and getting data but as of yet I am unsure how to generate a second email with the data collected.
After creating a group and some fields in the “Custom Fields” area of the downloads menu I went to add them to the form lock only to find that it would not show the field group I created in the drop down… it just said “Select an option” with nothing to select.
Ok… There was an issue someone else had that was similar and 4.5.0 had come out.
After update I was getting an error (see other post) so I deactivated WPDM until I get a resolution to that.
I will keep people updated on how I try to deal with this simple function that should be basic but is not.
@Substance151 Care to detail a guide on using the custom fields to deal with this? It apparently came with the pack I bought. I went to this plugin because it was supposed to be easy to setup an mange for non-coders
@substance151 I am even more frustrated because I asked this before I purchased.
I’d like to think what I want is simple.
Customer wants a download (Demo or otherwise), fills out a form, receives an email (that we can customize to look professional), and clicks the link in the email.
At the same time we get an email with all the info in the form and everyone is happy.
I bought the Pro Pack (seemed like a good deal at the time) . I will check to see if the custom fields addon is included.

Highly disappointed because I asked this very question before purchase and I was told that combining form and email was possible.
