First one was straight-forward.

Second:
I am using [wpdm-all-packages items_per_page="5" jstable=1] to display all packages. In this case icons are automatically displayed when I do bulk upload but not for individual.

I also tried [wpdm_packages link_template="link-template-default-icon" order_by="post_title" order="asc" paging="1" items_per_page="5" title="" desc="" cols=1 colsphone=1 colspad=1] using different link templates(also tried custom template with [icon]), but it always shows me same result(see attachment).
