Upload file does not "stick" after file uploads

I use this plugin on a number of sites, both the free and paid version. By far the best! But now I am having an issue. When I create a new package, upload the file everything seems to work fine. But after it completes, the file I uploaded does not appear within the package and the icon I selected does not stay…it goes back to the default icon. I deleted and reinstalled the plugin numerous times. Still does not work. I checked the file permissions on my server and they are all set correctly. Please help!
