When adding a translated PDF in downloadmanger i need to change the shortcode in WordPress.

Do i need downloadmanager Pro to add translations or do i need to change the shortcodes trougout the website?

Please advise!

Yes, it is same in pro too. But pro version have option to add multiple files per package, so you can add all PDFs from different languages in same package and users will be able to download the file in their preferred language.
