File Icon Not Appearing / Random Character in Title

Hi
When I select a file icon it does not appear in the listing it only shows this generic icon. Also, a random square is appearing next to the number the number of files. How do I fix this?

The update fixed the square next to the number of files but Download Manager created a conflict for my social icons on the rest of the site. I’m getting blank squares where they use to be. I have Disable Font Awesome checked in the settings but it doesn’t make a difference. Any help on this?
