How do I edit the contents of the download email? Also, where do I edit the download form fields? I’ve looked everywhere except the right spots I guess.

you can edit content of download email from “Downloads Subscribrs Email Template ( http://www.wpdownloadmanager.com/doc/subscribers/ )

editing form fields will be little complex and to lengthy to explain here, anyhow it is related with several functions at functions.php
