WP Download file is set up using Lock Options, Must Agree with Terms. The file settings work perfectly in the WP Downloads Preview.
When the shortcode is inserted into my Avada Theme, the dialogue box pops up, but the Must Agree with Terms is not visible. The Download button is visible, when activated it downloads a different file that what was set in the file/with shortcode.
Several weeks ago everything was working fine.
Please, give your temporary wp-admin login details in a private reply for checking the issue.

I have tested here, https://www.biblicalworld.org/wpdm-test by creating a sample package. and it is working fine here. please check
