Use category id separated by comma in category parameter value. Check following example,

You need Form Lock addon http://www.wpdownloadmanager.com/download/wpdm-form-lock/ if you want to use ninja forms in email lock.

In package setting check ‘Allow Access’ field. Maybe the user tring to download the file does not have access to it. However, you can send temporary wp-admin login info in private reply , so i can check this issue there.

That should change the box background color.

Somehow in your installation, authentication cookie is not being set when user login from frontend form. That’s what creating the issue. If you have any staging server try this setup there to be sure if this issue is related to some settings in your current server.

For now, found a workaround.  Use http://frtimer.com/wp-admin as login page. User can set featured image if logged in through this page. I will check further in this issue and will let you know the update.
